The Clinical Pharmacy Program Coordinator assists the Director in planning and implementing clinical pharmacy activities.
Implements and conducts continuing education programs for nurses, technicians and pharmacists.
Participates in hospital related community health education programs.
Prepares agenda, minutes, drug monographs, policy & procedures and formulary recommendations for the P&T Committee.
Provides consultation to physicians on drug usage, posology, and therapeutic appropriateness decisions.
Develops and monitors medication use and reports results to P&T Committee.
Assists with reporting P&T recommendations to MECThe value you'll bring to the team:
Coordinates, audits, and maintains existing and new pharmacy programs, quality improvement changes, and medication stewardship programs to advance the organization's medication-use systems and promote optimal patient outcomes.
Coordinates and maintains existing education and training programs designed to fully develop the skills of the pharmacy department staff and ensure the competency of the pharmacists involved in any pharmacy services.
Develops and coordinates educational content for medical staff members, nursing partners and ancillary teams.
Facilitates the development and implementation of treatment guidelines, protocols, formulary changes, and critical pathways as approved by appropriate committees.
Provides departmental and institutional representation for medical staff committees/meetings, hospital groups/task forces, and approved external programs/organizations.
Audits and coordinates pharmacy quality assurance program including pharmacy department compliance with healthcare regulatory and accreditation agency requirements, and applicable state and federal laws across the division.
Design, implement, and coordinate roll out of new pharmacy programs, quality improvement changes, and continuous quality assurance initiatives to advance the organization's medication-use systems and promote optimal patient outcomes.
Prepare agenda, minutes, drug monographs, policy & procedures and formulary recommendations for the P&T Committee.
Review utilization of formulary and non-formulary medication, including but not limited to adherence to criteria for use.
Design and maintain technology functions to optimize pharmacy operations, clinical services, automation devices and applications, medication safety, supply chain management, and pharmacy finance processes.
Maintain expertise in data analytics, project management, pharmacy automation hardware and applications, and electronic health record (EHR) integration and functionality.
Facilitate project management of pharmacy technology solutions to allow for smooth implementation, coordinated upgrades, proactive maintenance optimization, and timely changes in workflow.
Utilize operational knowledge of key medication safety technologies including medication dispensing systems, IV pumps, barcode medication administration, and computerized provider order entry to evaluate medication variances and improve processes.
Develop strategies to minimize the potential for medication errors associated with medication labeling, packaging, nomenclature, storage, standardization, preparation, distribution, dispensing, and administration.
Maintain expertise in the standards of The Joint Commission (TJC) and/or Det Norske Veritas (DNV), Board of Pharmacy, and other regulatory bodies relevant to Pharmacy Operations.
QualificationsThe expertise and experiences you'll need to succeed:
Minimum of Bachelor of Science degree in Pharmacy from an accredited school.
Doctor of Pharmacy degree preferred.
Currently licensed as a Clinical Pharmacist in the state of Florida.
